Responsibilities
- Provide skilled physical therapy tailored to individualized care plans
- Evaluate patient mobility, progress, and treatment effectiveness
- Help patients regain functional independence in their home environment
- Educate patients and caregivers on safe exercises and mobility techniques
- Maintain accurate documentation in EMR system (training provided)
- Collaborate with interdisciplinary care team to enhance outcomes
- Ensure compliance with policies, safety standards, and clinical best practices
Qualifications
- Active California Physical Therapist (PT) license
- Minimum 1 year of clinical physical therapy experience (Home Health preferred)
- Knowledge of home-based rehabilitation best practices
- Valid California Driver’s License and reliable transportation
- Professional communication skills and patient-centered care approach
